Job Summary
Develop and deliver personalized plans of care for diverse patients with orthopedic and neurologic diagnoses using cutting-edge software. Perform telehealth visits and Remote Therapeutic Monitoring (RTM) services, including interactive communication and outcome measurement. Efficiently document evaluations, treatments, and discharge notes while maintaining regular reporting of regional service metrics. Serve as a liaison between providers and the Director of Digital Health to optimize remote services and participate in program development and marketing efforts. Work 3 days in clinic and 2 days remotely in a hybrid role supporting operations to meet patient care and staffing needs.
Required Qualifications
- Must be located in Camden, SC (or within driving distance)
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license in Florida (or in process)
- Minimum 2 years of relevant clinical experience (Outpatient)
- Dedication to providing exceptional quality of care to each of your patients
Desired Qualifications
- Experience providing digital health services preferred
