Complete nutrition assessments for geriatric residents and develop, monitor individualized nutrition care plans. Participate in care plan meetings and interdisciplinary collaboration while ensuring compliance with CMS and NYS DOH regulations. Provide nutrition education to residents and families, support quality improvement and audit initiatives, and document all activities accurately. Required Qualifications
- Certified Dietitian - Nutritionist (CDN) License Required
- Registered Dietitian (Full-time)
- Registered Dietitian with NY license
