Dietitian-PT

Conduct clinical assessments, update patient care plans, and complete MDS sections while maintaining a working knowledge of the Medicare PDPM system. Document in PointClickCare and use meal planning software to ensure proper patient dietary needs. Collaborate with chefs and the dietary department to promote optimal food service operations, attend meal services, monitor resident intake, and assist with inservice training. Balance nutritional care with cost control by managing supplement usage. Communicate with physicians, families, and the interdisciplinary team regarding diet liberalization, weight changes, and new orders.
