Job Summary
Conduct clinical nutrition assessments, document findings in the EMR using the Nutrition Care Process, and complete MDS Section K evaluations. Collaborate with the interdisciplinary care plan team to make recommendations regarding wounds and significant weight changes, including TF calculations. Requires CDR credentialing, dietetic licensure, and strong communication skills. This 1-week temporary coverage (October 6-9) offers 6-8 hours weekly at Nutrition that Works, LLC's Houston long-term care community.
