Job Summary
Implement initial and annual nutrition assessments for frail elders, documenting diet orders, lab results, and medical conditions in clinic or home settings. Develop individualized nutrition plans and communicate them to participants, families, and interdisciplinary teams. Provide customized nutrition education and one-on-one guidance while supervising Dietary Aides on kitchen operations, food safety, and meal temperature compliance. Arrange home-delivered meals and specialized supplements as needed, ensuring all programs meet PACE, DPH, and CMS regulatory requirements. This role supports Harbor Health's Elder Service Plan, serving over 580 community-dwelling elders through comprehensive healthcare and social services.
Required Qualifications
- Bachelor's degree from an accredited college with major studies in Food and Nutrition or Dietetics
- Master's degree
- Current License in the state of Massachusetts as Registered Dietitian
- Current BLS (Basic Life Support for Healthcare Providers) Certification
- Intermediate skills with Microsoft Office 365 including Word, Excel, Outlook and Teams
- One year of professional experience providing nutrition counseling and assessments
- One year of experience working in geriatrics
- Excellent written and oral communication skills
- Current ServSafe – Allergen OR MA-Allergen Certification or obtained within 3 months of employment
- Current ServSafe Food Handler Certification (Food Handlers or Serve Safe Manager) or obtained within 3 months of employment
- Must be able to travel to and from multiple patients' homes and the work site in a timely manner
- Requires frequent movement including but not limited to bending, stooping, stretching, reaching, walking, sitting, pushing pulling for long periods of time
- Requires the ability to assist a falling participant
- Must be able to push participants in a wheelchair up to 300 lbs
- Must be able to climb stairs, manure narrow hallways, hear faint noises, and read small print
