Job Summary

Conduct nutrition assessments and reassessments for long-term care and rehabilitation residents while developing, implementing, and monitoring individualized nutrition care plans. Provide nutrition education and counseling to residents and families, and monitor nutritional status, weight trends, and overall wellness. Collaborate with Nursing, Dining Services, Therapy, and medical providers in interdisciplinary care plan meetings. Maintain accurate documentation within the electronic medical record system, perform nutrition-related audits, and support survey readiness. Assist with staff education and in-service training programs. Ensure compliance with CMS, DOH, and nutritional service regulations.

Required Qualifications

  • Bachelor's degree in Nutrition, Dietetics, or related field
  • Registered Dietitian credential required
  • Active registration with the Commission on Dietetic Registration (CDR)
  • Certified Dietitian-Nutritionist (CDN) or state licensure, as applicable
  • Intermediate or more advanced level English language proficiency (read, write, speak, understand)

Desired Qualifications

  • Long-term care, rehabilitation, or geriatric experience preferred
  • Experience with nutritional software and electronic medical records preferred
