Job Summary
Develop and implement comprehensive nutrition care plans for cardiac rehab patients, including those with MI, valve surgery, and heart failure. Provide individual and group education on therapeutic diets and blood glucose monitoring while documenting assessments, therapy recommendations, and discharge planning. Mentor new staff, participate in clinical care forums, and assist leadership in program planning and efficiency proposals. Ensure compliance with American Heart Association and AACVPR guidelines while collaborating with nurses, therapists, and behavioral health teams. Work per diem for 4 hours weekly at Newton-Wellesley Hospital.
Required Qualifications
- Bachelor's Degree Clinical Dietitian
- Master's Degree Dietitian
- Dietitian and Nutritionist [State License]
- Registered Dietitian [RD/RDN]
- Commission on Dietetic Registration (CDR)
- Clinical Dietitian experience with multiple patient populations
- 3-5 years
- Advanced knowledge pertaining to the evaluation, design, and standards of nutrition care for patients who qualify for or are enrolled in cardiac rehabilitation
- Ability to provide education to individuals and groups on nutrition
- Ability to function as an essential member of a multidisciplinary team providing special expertise to patients and families participating in the Cardiac Rehabilitation program
- Ability to develop and implement nutrition plan of care for patients based on assessment of nutritional needs, physician's orders, clinical data, interdisciplinary health care plans, and standards of quality nutritional care
- Ability to document all nutrition related information including assessment, nutrition therapy recommendations and care plan, follow up data and changes, diet consultation results, discharge planning, daily clinical activities, and other related orders and pertinent information
- Ability to counsel, educate, and develop resources and materials for the education of patients on families on prescribed therapeutic diets
- Ability to instruct patients to properly monitor blood glucose
- Ability to provide education to patients individually and in a group or classroom sitting
- Compliance with American Heart Association, American College of Cardiology, and American Association of Cardiovascular and Pulmonary Rehabilitation guidelines
