Job Summary
Dietitian - Adult in Ontario providing in-home and community-based nutrition care. Responsible for assessing and providing consultation, recommendations of dietetic interventions to adults (geriatric and/or pediatric clients in their homes) with coordinated delivery of care through ongoing communication with the client care team. Develops client reports, plans and presentations for clients and caregivers, educates internal and external staff, and supports interventions across failure to thrive, dysphasia, feeding problems, enteral feeding, diabetes, cancer, GI disorders, weight management, and palliative care. Emphasis on evidence-based practice, independence, and DEI commitments. Required Qualifications
- Member in good standing with the College of Dietitians of Ontario
- Minimum 2 years clinical experience
- Valid Driver’s License with access to a vehicle
- Able to provide virtual care
- Compassionate and empathetic
- Self-driven with ability to work independently
- Skilled in caseload management
- Skilled at electronic documentation
