Job Summary
Provide care following the National Standard for Diabetes Self-Management Education and Support by assessing patient needs, developing care plans, and educating individuals on diabetes treatment, nutrition, exercise, and medication management. Adjust diabetes medications and monitor insulin pump hookups under physician orders, while serving as a resource to medical staff, patients, and the community through formal instruction and support group coordination. Evaluate educational barriers, integrate health promotion, and participate in program development and the Diabetes Advisory Board Committee. Work a 36-hour week Monday through Friday from 8 AM to 5 PM, with hours determined by patient needs.
Required Qualifications
- Registered Nurse license issued by the state in which the team member practices
- Dietitian certification issued by the state in which the team member practices
- Dietitian (RD) registration with the Academy of Nutrition and Dietetics
- Certified Diabetes Care and Education Specialist (CDCES) certification issued by the Certification Board for Diabetes Care and Education (CBDCE) needs to be obtained within 3 years
- Bachelor's Degree (or equivalent knowledge) in Nursing
- Bachelor's Degree (or equivalent knowledge) in Dietetics
- 2 years of experience in a registered nurse or dietitian role with advanced clinical and technical knowledge of Diabetes
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served
- Must demonstrate knowledge of the principles of growth and development over the life span
- Must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs
- Must be able to provide the care needed as described in the department's policies and procedures
- Must maintain credentialing and continuing education requirements for recognition and representation of accredited DSMT program
- Must maintain a high degree of professional competence by ongoing education and membership in professional organizations
- Must have functional speech, vision and hearing
- Must be able to sit, stand, walk, lift, carry, push/pull, bend, squat, twist, rotate, kneel, and reach occasionally throughout the workday
- Must be able to lift 10-25 lbs. occasionally when transporting education materials
- Position may require local travel
Desired Qualifications
- Dietician with CDCES certification preferred
