Assess and monitor residents' nutritional status, completing thorough nutritional assessments upon admission, quarterly, annually, and with condition changes. Develop, implement, and routinely update nutritional care plans while monitoring for weight changes, nutrition support needs, and skin breakdown to provide timely medical recommendations. Oversee diet orders, review menu changes for compliance, and observe meal services to ensure appropriate therapeutic modifications and correct textures are served. Partner with the interdisciplinary team to align diets with specific medical conditions and provide essential nutritional education to residents, families, and staff. Perform regular inspections of food service areas for sanitation and safety, conduct routine care audits, and actively participate in facility inspection surveys.
