Job Summary
Provide direct patient care activities including assessment, diagnosis, planning, implementation, and evaluation for Acute Dialysis, Hemodialysis, and Peritoneal Dialysis patients. Promote patient and family-centered care by educating families on illness management, post-treatment home care needs, and medication administration. Develop and document individual plans of care with defined goals in collaboration with the interprofessional team while ensuring regulatory compliance and patient safety. Assess physical, psychosocial, and emotional needs according to standards of care, including patient preferences and barriers to involvement. Work in a constant state of alertness as a safety-sensitive position with a 2:1 patient ratio on 12-hour shifts.
Required Qualifications
- Prior dialysis experience
- Acute Dialysis
- Hemodialysis
- Peritoneal Dialysis
- Nursing Diploma/Associate's degree
- RN license
- Full-time employment
- 36 hours per week
- 12-hour shifts: 10:00 AM to 10:00 PM
- One weekend per month
- Occasional weekend call coverage
- Ability to work in a constant state of alertness
- Assessment and evaluation of physical, psychosocial, and emotional needs
- Assessment of patient preferences and barriers to involvement in care
- Communication of plan of care across the continuum of care
- Patient and family centered care
- Education of patients and families on illness management, post treatment home care needs, and medication administration
- Promotion of patient safety, quality, and regulatory compliance
- Participation in professional development
- Development, implementation, and documentation of individual plans of care
- Collaboration with interprofessional team, patient, family, or caregiver
- Adherence to established guidelines and standards of nursing care
