Job Summary
The Home Therapy Registered Nurse provides patient training and ongoing support for all patients choosing a home dialysis modality (home hemodialysis or peritoneal dialysis), with responsibilities including patient and care partner training in self-care, conducting home visits, documenting nursing services in the EMR, participating in infection control and emergency procedures, and coordinating supplies and medication reviews. Requires current RN license in the applicable state, 12 months of clinical nursing experience with at least 3 months in the specific home dialysis modality, CPR certification, English proficiency, and computer skills with USRC systems. Schedule is Monday–Friday 8am–5pm with on-call rotation and potential for flexible staffing locations; this is a full-time role based in the Houston, TX facility. The role emphasizes improving patient outcomes through data tracking, quality improvement activities, and collaboration with physicians and the care team.
Required Qualifications
- Current RN license in applicable state
- 12 months of clinical experience in providing nursing care and 3 months of experience in the specific modality for which the nurse will provide home dialysis training
- CPR certification required prior to patient care assignment
- Demonstrated working knowledge of the English language
- Must have basic computer skills; proficiency in all USRC clinical applications required within 90 days of hire
