Job Summary
Home Therapy Registered Nurse trains patients (and/or care partners) in self-care for Peritoneal Dialysis or Hemodialysis, in-center or at the patient’s home. Responsibilities include conducting home visits, evaluating home environments, documenting nursing services in the EMR, coordinating infection control measures, ensuring adequate supply inventory, and supporting quality outcomes through data tracking and routine communication with physicians and care teams. Requires current RN license, CPR, and modality-specific nursing experience; on-call rotations and flexible staffing locations. The role emphasizes patient training, clinical care, and adherence to federal/state regulations to support home dialysis programs.
Required Qualifications
- Current RN license in applicable state
- CPR certification
- 12 months of clinical nursing experience
- 3 months experience in the specific modality (home dialysis)
- Preferred: prior nursing care to dialysis patients
- Ability to read, write, and communicate effectively in English
- Basic computer skills and proficiency with clinical applications within 90 days of hire
- Must meet state practice requirements
- Ability to work on-call nights and weekends as needed
