Job Summary
Home Therapy Registered Nurse travels to Renal Care locations to provide patient training and ongoing support for home dialysis modalities, serving on a temporary/short-term basis. Responsibilities include educating patients and/or care partners in self-care for Peritoneal Dialysis or Hemodialysis, coordinating with physicians and interdisciplinary teams, tracking patient outcomes, maintaining documentation in the EMR, participating in infection control activities, and supporting program goals. The role may involve home visits, on-call nursing coverage including nights and weekends, and travel across the U.S. and Guam to support multiple home dialysis programs.
Required Qualifications
- Current RN license in multiple states
- 12 months or more current nursing experience
- 6 months or more home modality experience required
- CPR certification
- Ability to meet state practice requirements
- Ability to use Electronic Medical Records
- Good English communication skills
- Willingness to travel throughout the U.S. and Guam
- Complete Home Therapy Nurse Orientation program
- Basic computer skills
