Job Summary

Coordinate daily hemodialysis therapy for assigned patients, including assessment, medication administration, and emergency response. Conduct pre-, during-, and post-treatment data collection and treatment plan modifications while documenting all care in medical records. Lead and mentor Patient Care Technicians, LVNs, and other staff under supervisor direction. Provide patient and family education on transplant options and treatment modalities. Ensure accurate laboratory collection, processing, and shipping procedures. Participate in facility and area-level quality improvement projects and serve as a preceptor for new employees. Requires 2 years of nephrology nursing experience and float pool dialysis experience.

Required Qualifications

  • Float Pool Dialysis Experience
  • RN
  • Entry level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria
  • Attained through advancement from RN CAP 1
  • Accountable and responsible for the provision and coordination of clinically competent care
  • Assessment, planning, intervention, and evaluation for an assigned group of patients
  • Delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs, and Patient Care Technicians
  • Participation in decision-making, teaching, leadership functions, and quality improvement activities
  • Ongoing, systematic collection and analysis of patient data pre – during – post hemodialysis treatment
  • Documentation in the patient medical record
  • Adjustments or modifications to treatment plan
  • Notification of Team Leader, Charge Nurse, Supervisor or Physician as needed
  • Assessment, collaboration, and documentation of patient/family's basic learning needs
  • Initial and ongoing education to patients and family
  • Daily provision of safe and effective delivery of hemodialysis therapy to assigned patients
  • Administration of medications as prescribed or in accordance with approved algorithm(s)
  • Documentation of appropriate medical justification and effectiveness
  • Initiation or assistance with emergency response measures
  • Serving as a resource, leader, coach, mentor and role model
  • Setting examples of appropriate behavior, work habits and attitudes
  • Ensuring correct laboratory collection, processing and shipping procedures
  • Rescheduling of missed or insufficient laboratory collections
  • Identification of expected outcomes
  • Documentation and updating of the nursing assessment and plan of care
  • Ensuring patient awareness related to transplant and treatment modality options
  • Participation in education and quality improvement projects
  • Serving as a Preceptor to new employees
  • Completion of CAP requirements
  • Minimum of 3 months experience in chronic/acute hemodialysis as a RN
  • Graduate of an accredited School of Nursing
  • Current appropriate state licensure
  • Current or successful completion of CPR BLS Certification
  • Meeting the practice requirements in the state in which he or she is employed
  • Entry level for RNs with minimum of 2 years of Nephrology Nursing experience
  • Ability to lift 50 lbs
  • Ability to lift chemical and water solutions of up to 30 lbs
  • Ability to lift machines and equipment of up to 200 lbs
  • Ability to stand for prolonged periods
  • Ability to bend over
  • Ability to move machines and equipment with assistance
  • Ability to move patients and assist with ambulation
  • Availability for travel to training sites or other facilities
  • Ability to provide essential functions of this position in other locations
  • Drug-free workplace compliance
