Job Summary
Provide and coordinate clinically competent home dialysis care for an assigned group of patients, including assessing and managing patients’ response to home dialysis training and treatment therapy, communicating issues to the physician, and participating in decision-making, teaching, leadership, and quality improvement activities. Responsibilities include education for patients and families, directing home dialysis care with staff and partners, administering medications per approved algorithms, coordinating with the interdisciplinary team, ensuring proper lab procedures, identifying outcomes and updating care plans, and supporting transplant and modality education. Requires completion of CAP requirements to advance, a nursing degree, state licensure, CPR/BLS certification, and home dialysis experience as noted. Travel to training sites or patient homes may be required and the role involves on-call rotations and rotating shifts.
Required Qualifications
- Graduate of an accredited School of Nursing
- Current appropriate state licensure
- Current or successful completion of CPR BLS Certification
- Must meet the practice requirements in the state in which he or she is employed
- Entry level for RNs with less than 2 years of Nephrology Nursing experience (preferred)
- Home dialysis therapy experience (preferred)
