Job Summary
Diabetes Educator-Nurse provides diabetes self-management education to outpatients and their families/caregivers in individual and group settings. Educates on diabetes disease process, nutrition, exercise, medications, glucose monitoring/meter use, insulin/injectable therapy, and insulin pump management; teaches and reinforces self-care concepts and behavior-change strategies; develops personalized education plans, guides patients in setting goals, coordinates with prescribers as needed, and maintains education records. Collaborates with multidisciplinary teams to promote quality care, participates in performance improvement activities, and ensures ongoing continuing education related to diabetes management.
Required Qualifications
- Education - Completion of a nursing education program that is approved by a board of nursing
- License - Registered Nurse (RN) - South Dakota Board of Nursing Certification - Certified Diabetes Educator (CDE) - National Certification Board for Diabetes Educators (NCBDE) preferred
- Experience - 1+ years of Diabetes Education Experience; 1+ years of Registered Nurse Experience
- Shift - Assigned schedules as determined by department
- Physical Requirements - Light work
