Job Summary
Coordinate patient diabetes self-management and nutritional care through clinical assessment, education, and documentation across inpatient and outpatient settings. Assess biological, emotional, and nutritional needs to develop, implement, and maintain individualized nutrition care plans. Provide specialized diabetic diet education to patients, families, and healthcare teams while fostering continuity of the Nutrition Care Process. Collaborate with interdisciplinary teams to establish DSMES goals, conduct chart audits, and participate in quality improvement initiatives. Serve as a diabetes resource within the staff and community, presenting to departments and civic partners while maintaining professional competencies through required CEUs.
