Job Summary

Coordinate the diabetes patient education program by serving as a liaison between the advisory committee and administration, planning orientation sessions for new personnel, and providing in-service education to staff. Design and evaluate program content, manage permanent patient records, and oversee instructional and clerical teams. Monitor DSMES team qualifications, maintain ADA recognition, and follow up with inpatient consults and outpatient referrals while systematically collecting patient health data to develop care plans. Work with the education team on assigned projects and maintain ethical confidentiality. Requires BSN or MSN, CDCES certification, and Wisconsin nursing licensure.

Required Qualifications

  • Registered nurse
  • Certified by the National Certification Board for Diabetes Educators
  • Broad range of knowledge of health and wellness
  • Experience in care of people with diabetes and chronic diseases
  • Graduate of accredited school of nursing
  • Bachelor's degree
  • Must obtain bachelor's degree within 4 years of hire
  • State of Wisconsin or multistate nursing licensure
  • Current IL licensure
  • Must request WI licensure within 30 days
  • Excellent oral and written communication skills
  • Ability to organize and prioritize multiple demands
  • Ethical conduct
  • Ability to keep confidential personal and medical information about patients
  • Ability to adhere to established health system and department policies, rules and regulations

Desired Qualifications

  • MSN
  • Minimum of three to five years' recent clinical experience
  • BLS certification within three months of hire
