Job Summary
Develop a positive relationship with individuals served and assist them in achieving personal goals as outlined in individual goal plans. Provide supports to help individuals pursue hopes and dreams, facilitate advocacy and involvement with community agencies, supervise and educate those receiving services in work settings (including community, vocational workshops, and day supports), and ensure safety and compliance. Maintain documentation (daily progress notes, logs, timesheets, incident reports), participate in meetings and trainings, and may require sleepovers at residential settings. Travel may be required; driving and travel are possible. Requires High School Diploma and valid driver’s license.
