Job Summary
Implement Day Habilitation Service Plan objectives and document progress in daily logs and quarterly reports. Assist individuals with meaningful activities, choices, hygiene, grooming, and physical needs including transfers and eating. Plan weekly schedules, transport for community-based habilitation, and ensure participation in therapeutic experiences. Maintain a safe, clean environment while safeguarding human dignity, rights, and confidentiality. Intervene in behavioral incidents and report daily activities and unusual incidents to the Manager. Complete datasheets for therapists and follow established communication chains.
Required Qualifications
- High School Degree or G.E.D.
- Must be 21+ years old
- Valid driver's license
- Daily use of a car on site
- Available by telephone for daily contact
- At least two years of experience in a training program for the developmentally disabled
- Ability to bend, stoop, reach and lift - physical flexibility
- Maintain First Aid, CPR, AED & Safety Care Certifications
- Good written and verbal communication skills
- Maintain training specific to individual programmatic issues
Desired Qualifications
- Daily use of a car on site (preferred but not required)
