Job Summary
Provide person-centered, applied support to individuals receiving services to help them pursue their hopes and desires, develop community relationships, and participate fully in daily life. Responsibilities include assessing goals, promoting advocacy, delivering emotional/physical/medical support as needed, documenting progress and incident reports, participating in planning processes, coordinating transportation, and assisting with home operations and safety. Requires a high school diploma, valid driver’s license, ability to work PRN/part-time shifts, and experience with populations served (mental health, intellectual and developmental disabilities, or substance use disorders). At Monarch, you’ll help people thrive, collaborate with a supportive team, and contribute to services across North Carolina and Rhode Island while upholding policies and training requirements.
