Job Summary
Assess each person receiving services for their hopes, dreams, and desires to provide supports that facilitate achievement. Support individuals in developing relationships within their community and assist them in fully participating in their community consistent with their interests. Provide support for emotional, physical, and medical needs while ensuring rights are protected and promoted. Implement the person's plan fully, document goal completion, actions toward outcomes, and incident reports accurately. Assist with day-to-day home operations including housekeeping, meal preparation, laundry, and transportation. Conduct fire safety drills, inspect equipment, and utilize effective judgment regarding safety issues. Complete daily progress notes, communication logs, and attend required meetings and training. Maintain certification in all agency, state, and federal training requirements.
Required Qualifications
- High School Diploma
- Valid Drivers License - USA
- Experience working with children or adults (dependent upon population served) with a mental health diagnosis, an intellectual/developmental disability, or substance use disorder
- Sleepover at a residential setting
- Must be available for Week 1: Mon 6am-4pm, Wed 4pm-10pm, Thurs 6am-10pm, Friday 6am-4pm
- Must be available for Week 2: Fri 4pm-10pm, Sat 6am-10pm, Sun 6am-10pm
- Must be able to drive and travel as required
- Must be able to assist people receiving services with the day-to-day operation of their home such as housekeeping, meal preparation, laundry, shopping, lawn care, vehicle cleanliness, and maintenance
- Must be able to assist people receiving services during late night hours on occasion
- Must be able to provide transportation to people receiving services as required
- Must be able to assist people receiving services with knowledge of emergency procedures and personal safety
- Must be able to conduct drills, inspect equipment, and practice safety procedures in the community
- Must be able to assist new staff and/or current staff with orientation, mentoring, and training
- Must maintain certification in all agency, state and federal training requirements
- Must demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas
- Must be able to accurately complete all required documentation, including but not limited to goal completion, actions toward outcomes, incident reports, timesheets, etc.
- Must be able to complete daily progress notes and communication log to assure appointments, goals, and interests are met
- Must be able to attend and actively participate in meetings and training as required
