Job Summary
Develop and implement person-centered supports for individuals with mental health diagnoses, intellectual/developmental disabilities, or substance use disorders in a residential/clinic setting. Assess each person’s hopes and desires, help them participate in community life, support relationship-building with natural supports, and promote self-determination. Provide input into assessments and individual plans, document progress and incidents, complete required records, and assist with day-to-day home operations, transportation when needed, and safety procedures. Sleepover may be required. Requires ability to drive and travel as needed, plus ongoing training and adherence to agency policies. This role supports Monarch’s mission to promote wellness and empower individuals and families through compassionate, community-based care.
Required Qualifications
- High School Diploma (Required)
- Driver's License (Valid)
- Experience working with children or adults with mental health diagnoses or developmental disabilities or substance use disorders
