Job Summary
Develops and delivers person-centered supports to individuals with mental health diagnoses or intellectual/developmental disabilities or substance use disorders. Responsibilities include assessing individuals’ hopes and goals, supporting community participation and relationships, assisting with daily living activities (housekeeping, meal prep, laundry, shopping), facilitating advocacy and self-determination, maintaining accurate documentation and progress notes, ensuring safety procedures, providing transportation as needed, and coordinating with staff for training and planning. Sleepovers may be required; documentation and compliance with service definitions and agency policies are expected. Requires a High School Diploma and a valid driver's license. Monitoring and supporting emotional, physical, and medical needs while promoting rights and community integration.
