Job Summary
Implement Day Habilitation Service Plan objectives and document daily progress in logs and data books. Assist individuals with activities, choices, hygiene, grooming, and physical needs like transfers and mobility. Plan weekly schedules, transport to community experiences, and provide hand-over-hand support while maintaining a safe, clean environment. Ensure health, safety, and well-being by following emergency plans and addressing behavioral incidents. Submit quarterly progress notes and incident reports to managers and therapists. Maintain confidentiality, uphold human dignity, and collaborate with families, staff, and community members.
Required Qualifications
- High School Degree or G.E.D.
- Must be 21+ years old
- Valid driver's license
- Daily use of a car on site
- Available by telephone for daily contact
- At least two years of experience in a training program for the developmentally disabled
- Ability to bend, stoop, reach and lift
- Physical flexibility
- Maintain First Aid, CPR, AED & PABC Certifications
- Good written and verbal communication skills
- Maintain training specific to individual programmatic issues
Desired Qualifications
- Daily use of a car on site (preferred but not required)
