Job Summary
Coordinate day-to-day programmatic requirements and resolve concerns for a designated caseload of children and adults receiving services. Conduct home studies with families, gather licensing paperwork, and create training curricula for foster families and support staff. Monitor licensed providers' care, review placement profiles, and advocate for quality placement relationships while ensuring adherence to licensing rules. Manage staff coverage to avoid service disruptions, maintain accurate documentation including medication logs and habilitation goals, and reflectively analyze family progress during home visits. Report emergencies within a half business day and placement disruption risks within two working days. Carry a caseload requiring local and out-of-county travel within Maricopa County.
Required Qualifications
- Bachelor's Degree within the related field of Human Services, or related fields
- Minimum of 2 years experience in the Human Services field
- Associate's Degree in a related field
- 5 years experience working with individuals with developmental disabilities
- 5 years experience working in the child welfare industry
- Knowledge of the company policies and procedures
- Knowledge and skill in working with individuals with developmental disabilities
- Knowledge of licensing and regulatory requirements
- Knowledge and skill in family-professional partnerships
- Training skills
- Valid Arizona Driver's License
- Reliable means of transportation with current insurance and registration
- Article Nine Training
- Arizona Fingerprint Clearance Card
- Central Background Clearance
- Three completed references
- Certification in Adult/Child/Infant CPR
- First Aid certification
- Ability to address more than one project at a time while prioritizing the demands and issues in a busy operating environment
- Possess excellent writing skills
- Must be familiar with Microsoft Word and Excel Office software
- The ability to teach small and large groups in a classroom setting
- The ability to effectively supervise Providers
- Possess organizational skills and the ability to multi-task
- Must have outstanding communication and social skills
- The ability to initiate tasks necessary for the program to run efficiently
- Possess professionalism at all times while representing Family Partners, L.L.C.
- Possess guidance skills and the ability to delegate responsibilities
- Ability to work well on multiple projects and under deadlines
- Must be able to work as a team
- Must be available to deal with emergencies outside of business hours when scheduled to share the On Call phone
- Must carry a cell phone (company provided) and be available to answer all calls during business hours
- Must be able to lift 50 lbs
