Job Summary
Developmental Disabilities Transition Specialist responsible for disseminating information, providing education, and engaging with the community to support individuals aged 14-21 with intellectual and developmental disabilities. Build partnerships with school districts, service providers, state agencies, and other entities to ensure access to inclusive supported employment opportunities and successful post-transition employment. Design projects and deliver presentations for professionals and families; stay informed on best practices, legislation, and resources related to transition services and employment supports. Collaborate with DDA and DVR, review IE/CI/CDS requests in the AWA system, facilitate outreach to families and schools, and participate in IEP meetings and related activities. Requires Bachelor's degree in Public Health or related field, two years of relevant experience, knowledge of RCWs/WACs and HIPAA, proficiency in English, and ability to travel within Yakima County. Must obtain or maintain public health immunizations and a valid WA driver’s license; commitment to equal opportunity employment and adherence to pay transparency provisions.
