Job Summary
Screen patients via phone and walk-in inquiries, initiating insurance verification and scheduling assessments. Complete all paperwork clearly and conduct daily facility rounds and follow-up calls per policy. Maintain HIPAA compliance while formulating discharge and aftercare plans with patient involvement. Perform community outreach telephone calls and complete bio-psychosocial assessments. Report daily census to corporate office and assist with patient tailored discharge planning for those completing treatment.
Required Qualifications
- Bachelor's degree in Human Services
- Minimum of 2 years of social service and/or clinical service work
- Knowledge of addiction and/or dependence
- Must be a team player
- Willing to accept and promote goals with minimal supervision
- Ability to assist and support others
- Ability to identify problems and implement solutions
- Ability to handle stressful situations
- Ability to interact with others
- Past experience in hospital setting
- Past experience in hospital management
- Satisfactory drug screen
- Criminal background check
Desired Qualifications
- Substance abuse counselor and/or case manager, previous experience in substance abuse withdrawal, MAT Treatment, or detox
