Job Summary
Lead programs that directly improve the quality of life for individuals receiving services by coordinating and overseeing support services, collaborating with leadership, DSPs, families, and support teams to ensure person-centered services are delivered safely and in compliance; design, implement, and monitor individualized support programs; train and mentor House Leads and DSPs; maintain compliance with Service and Support Plans, policies, and regulatory standards; provide coaching and ongoing support to team members; maintain accurate documentation; identify opportunities for improved service delivery and positive client outcomes; respond to changing needs and operational priorities; perform additional duties as outlined in the full job description.
Required Qualifications
- Must successfully pass a Minnesota DHS Background Study (MN 245D)
- Valid driver’s license
- Reliable, insured transportation
- Strong communication, organizational, and computer skills
- Ability to effectively lead, mentor, and support Direct Support Professionals
- Education & Experience Requirements:
- Bachelor’s degree in a human services-related field and 1 year of direct care experience
- Associate degree in a human services-related field and 2 years of direct care experience
- Diploma in a human services-related field and 3 years of direct care experience
- OR Minimum of 50 hours of human services/disabilities-related training AND 4 years of full-time direct care experience under appropriate supervision
