Required Qualifications
- DDS/DMD from an accredited dental education program (CODA)
- Current, valid dentistry license in states of practice or eligible for licensure

- Compassion and desire to treat children and adults
- Willingness and ability to travel to listed locations in Indiana (travel within geographic area)
