Dental Case Manager

Dental Case Manager responsible for facilitating dental case management initiatives and deployment operations, including coordinating with Medical Records to ensure timely and accurate processing. duties include intake services at PHA and Medical Readiness, identifying medically non-deployable members, creating and tracking referrals in e-case, managing Soldier medical/dental care from injury/condition identification to return to duty or separation, initiating case management and maintaining clinical notes referencing providers’ medical assessments, performing SM reviews for patient care quality, coordinating continuity of care plans and all referrals when government resources are used, ensuring completion of checklists and proper handling of dental/medical records in HRR, documenting activities in Army National Guard electronic data systems, telephoning Soldiers and medical/dental offices to coordinate appointments under DHA-GL approved care, obtaining releases of information, placing and maintaining medical documentation in HRR and MRR, coordinating preauthorizations via MEDCHART, ensuring readiness issues are addressed with SM, ensuring all necessary medical documentation supports treatment approvals, providing non-clinical SRP and medical readiness event support, attending administrative support at readiness events and CH3 Evaluations, providing weekly updates/reports of case management activities, coordinating care with TRICARE and DHA-GL, and documenting bi-monthly care plans and ensuring timely data entry. Requirements emphasize strong organizational, written and oral communication, ability to manage priorities, and strong administrative skills.
