Job Summary
Conduct initial assessments to determine program eligibility and suitability for individuals living with dementia and their carers. Provide personalized guidance, support, and referrals to address specific skill gaps, and develop individualized care plans incorporating education sessions where required. Act as a vital link connecting participants with necessary resources to maximize wellbeing. Maintain accurate information while adhering to data security and privacy standards. Requires a current Queensland driver's license and relevant community services qualifications.
Required Qualifications
- Minimum 5 years' experience supporting individuals living with dementia and their carers
- Certificate of attainment in CHCAGE011 – Provide Support to People Living with Dementia
- Current C or CA class Queensland driver's license
- Solid understanding of dementia, common symptoms, subtypes, and impacts on individuals and carers
- Ability to work with clients, colleagues, and stakeholders while managing information accurately
- Competent in Microsoft Office suite
- Australian Federal Police check
- NDIS disability worker screening check
- Blue Card for working with children
Desired Qualifications
- Diploma of Community Services
- Certificate IV in Community Services
- Similar qualifications to Diploma of Community Services or Certificate IV in Community Services
- Willingness to obtain CHCAGE011 – Provide Support to People Living with Dementia
