Job Summary
Conduct initial assessments to determine program eligibility and suitability for individuals living with dementia and their carers. Provide personalized guidance, support, and referrals to address specific skill gaps, while developing individualized care plans that incorporate education sessions where required. Act as a vital link connecting participants with necessary resources to maximize wellbeing and quality of life. Maintain accurate information management and adhere to data security and privacy standards. Requires minimum 5 years' experience supporting dementia carers, CHCAGE011 certificate, and a current Queensland driver's license.
Required Qualifications
- Minimum 5 years' experience supporting individuals living with dementia and their carers
- Certificate of attainment in CHCAGE011 – Provide Support to People Living with Dementia (or willingness to obtain)
- Current C or CA class Queensland driver's license
- Solid understanding of dementia, common symptoms, subtypes, and impacts on individuals and carers
- Ability to work with clients, colleagues, and stakeholders while managing information accurately
- Competent in Microsoft Office suite
- Able to adhere to data security and privacy standards
- Work independently
- Communicate openly
- Demonstrate solution-focused thinking in a dynamic environment
Desired Qualifications
- Diploma of Community Services, Certificate IV in Community Services, or similar
