Job Summary
Dementia Consultant at HammondCare (Dementia Support Australia) on a Fixed Term Full-Time Contract based in Daw Park. Deliver dementia support programs, coach carers and clinicians, develop evidence-based resources and behaviour support planning, synthesize observations into contextualised recommendations, conduct holistic assessments, and collaborate with the Dementia Support Australia network to improve outcomes for people living with dementia. Roles involve reporting, project work, and contributing to research and program development; travel to support home and community-based care; opportunities for professional development and a comprehensive benefits package. Must have a current Australian driver’s licence and be willing to travel.
Required Qualifications
- Current Australian drivers licence and willingness to travel
- Relevant qualification in Allied Health, Nursing, Aged Care or Mental Health is essential
