Delta: Care Coordinator

Coordinate care for home health clients across Western Colorado: manage an assigned caseload, perform intake assessments, coordinate with clients, families, caregivers, and community partners, support caregiver scheduling and coverage, complete documentation, and communicate with families, providers, and partner agencies to ensure high-quality, compassionate care. Travel up to 50% and work from home with occasional in-office meetings and client visits. Strong communication, organizational, and problem-solving skills; ability to work independently, in a fast-paced environment, and as part of a team.
