Job Summary
Partner with Registered Nurse Case Managers to support members through assessment, care planning, and advocacy. Identify and coordinate community resources related to housing, transportation, financial assistance, safety, and behavioral health needs. Engage members telephonically and electronically to support positive health and lifestyle outcomes while collaborating with interdisciplinary teams to address medical, psychosocial, and behavioral health needs. Assist members in overcoming barriers to care and accessing appropriate services across all stages of life. This role requires a Master's Degree in Social Work and active unrestricted licensure, with consistent weekday hours from Monday–Friday, 8:00 AM–4:30 PM EST.
Required Qualifications
- Master's Degree in Social Work
- Active, unrestricted licensure as one of the following: LSW (Licensed Social Worker)
- Active, unrestricted licensure as one of the following: LMSW (Licensed Master Social Worker)
- Active, unrestricted licensure as one of the following: LCSW (Licensed Clinical Social Worker)
- Active, unrestricted licensure as one of the following: LISW (Licensed Independent Social Worker)
- 2+ years of experience in medical social work or case management
- Ability to obtain and maintain multi-state licensure (minimum of 10 states – willing to obtain additional licenses) before completing new hire training
