Job Summary
Conduct comprehensive evaluations to identify patient needs and set realistic rehabilitation goals for individuals recovering from surgery, managing chronic conditions, or dealing with mobility impairments. Develop individualized treatment plans utilizing manual therapies and exercise-based interventions to enhance mobility, reduce pain, and promote functional independence. Educate patients on adherence to therapy programs and monitor progress through diligent assessments to adjust treatments as necessary. Maintain detailed documentation of patient encounters using EMR software, collaborate with interdisciplinary teams including physicians and nurses, and encourage patients through their rehabilitation journeys. Work onsite in patients' homes Monday through Friday from 9 AM to 5 PM.
Required Qualifications
- A Doctorate in Physical Therapy (DPT) from an accredited program
- An active Physical Therapist license in the state of New Jersey, with no history of sanctions
- Proven expertise in various manual therapy techniques and evidence-based treatment methods
- Strong skills in developing and implementing effective exercise programs tailored to patient needs
- A commitment to staying current with advancements in physical therapy practices and research
- Minimum of two years of professional experience in a physical therapy setting
Desired Qualifications
- Familiarity with electronic medical records systems, particularly WebPT
- Experience in home care or outpatient rehabilitation services
- Familiarity with working independently in a community setting
