Job Summary
Conduct thorough evaluations and assessments of patients referred by physicians, developing individualized treatment plans that align with their medical needs. Implement diverse therapeutic interventions including orthopedic rehabilitation, neurologic therapies, sports medicine, and pediatric care while delivering high-quality, evidence-based care. Document patient progress and adjust care plans as necessary, ensuring consistency with physician directives. Educate patients and their families on exercise regimens, mobility strategies, and self-care practices to foster independence. Collaborate with other healthcare professionals to optimize the delivery of patient-centered care and achieve maximum effectiveness in treatment outcomes. Meet established performance targets and contribute to the overall success of the clinic through effective patient management and service excellence.
Required Qualifications
- A Doctorate in Physical Therapy (DPT)
- Current, valid State License in Physical Therapy
- Minimum of two years of experience in a clinical setting
- Basic Life Support Certification from the American Heart Association
- Demonstrated proficiency in verbal and written communication
- Adeptness in using electronic medical records (EMR) and other health technology platforms
- Strong interpersonal skills to build rapport and trust with patients
