Conduct thorough patient assessments to determine functional limitations and establish individualized care plans in collaboration with the patient and healthcare team. Implement therapeutic interventions promoting recovery using evidence-based practices, including exercises and manual therapy techniques to restore function and reduce pain. Educate patients, families, and caregivers about therapeutic strategies to facilitate ongoing recovery beyond sessions. Regularly evaluate and document patient progress, adjusting treatment plans as necessary to foster continuous improvement. Manage a caseload effectively by prioritizing tasks and coordinating with multidisciplinary team members. Provide direct physical therapy services to adults and pediatric clients in their homes, focusing on orthopedic, neurological, and chronic illness rehabilitation. Work Monday through Friday from 2 PM to 8 PM.
