Job Summary
Conduct thorough patient examinations to develop evidence-based treatment plans tailored for recovery. Collaborate with interdisciplinary teams through care plan meetings and contribute insights on patient progression. Educate patients and families regarding conditions, therapy processes, and home exercise programs. Document daily progress notes accurately in compliance with legal and facility standards. Maintain continuous communication with supervising Physical Therapists to ensure seamless continuity of care. Participate in facility in-service training to enhance understanding of therapy services among staff. Work primarily with elderly individuals recovering from surgeries, residents with chronic conditions, and those requiring post-injury rehabilitation in an outpatient office setting within a skilled nursing facility.
Required Qualifications
- A Bachelor's Degree in Physical Therapy or a related field
- Current, valid Physical Therapist license in Virginia in good standing
- Current CPR certification
- Possession of a valid driver's license and automobile insurance
- Ability to demonstrate advanced critical thinking and problem-solving skills in the clinical environment
- Strong interpersonal skills for effective communication with patients, their families, and team members
Desired Qualifications
- A Doctorate in Physical Therapy
- Prior experience in a Skilled Nursing Facility
- Demonstrated success in developing and implementing rehabilitation programs that align with patient goals and improve therapeutic outcomes
