DDS Residential Counselor - Creek Street

Assist in developing individualized, person-centered treatment plans and lead daily implementation plans for adults with developmental disabilities. Implement procedures ensuring the health, safety, and well-being of individuals while maintaining general cleanliness and safety of the physical site. Provide guidance and support for activities of daily living, help individuals access transportation, and encourage active community participation through volunteering, classes, and outings. Review and complete all required documentation. This role offers an additional hourly differential based on skills, location, training, and population served. Open Sky supports individuals referred by the Department of Developmental Services to live independently within a group living environment in Central Massachusetts.
