Job Summary
Coordinate case management for DDS-funded residential participants and other funded individuals, developing and monitoring annual service plans while ensuring compliance with CARF and federal, state, and local regulations. Assist participants in accessing community resources, schedule medical appointments, and communicate with families, advocates, and staff to ensure customer satisfaction. Perform monthly record reviews, complete required documentation, and supervise staff to maintain accurate schedules and promote full community integration. Participate in rotating on-call rotations for after-hours and weekend emergencies, submitting reports timely. Requires a Bachelor's degree in Human Services or related field, minimum one year of experience in direct care or case management, and current Connecticut driver's license.
Required Qualifications
- Minimum 1 year of experience in direct care, case management or human services field
- An equivalent combination of education and experience
- Excellent verbal, written, organizational and administrative skills
- Solid computer application skills
- Current Connecticut driver's license and daily use of a reliable, insured vehicle
- Willingness to obtain and maintain medication certification, CPR and PMT certifications
- On-Calling Rotating Scheduled Required
- Under the HIPAA Privacy Rule, this position has access to program participants' Protected Health Information (PHI)
- Oversee and ensure compliance with HIPAA, CARF and all other relevant federal, state and local regulations
