Job Summary
Conduct comprehensive assessments of clients and document needs to develop service plans. Arrange and monitor delivery of services through referrals, tracking appointments, and periodic reassessments. Perform advocacy activities by writing letters to expedite payments and assist clients in accessing resources. Provide case consultation with service providers and deliver psychosocial education to clients and families. Execute crisis assistance including emergency referrals and coordination of urgent services. Maintain case records in accordance with SCS and regulatory standards. Communicate with supervisors regarding resourcing and service collaboration. May serve as Primary Case Coordinator based on skill and autonomy. Requires Bachelor's degree in Social Work or related field and working knowledge of SCS programs. Includes 24-hour on-call responsibilities.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or related field as approved by Medicaid, or as required by applicable funding source
- Minimum of zero year's experience in case management/service coordination with a Bachelor's degree
- Working knowledge of mental and physical characteristics of assigned population group (e. g., growth and development), human behavior, treatment/service options, and related issues
- Working knowledge of SCS programs and services, policy, and procedure
- Working knowledge of applicable regulations and licensure standards
- Good interpersonal skills and written and oral communication skills
- Position requires lifting of 20 pounds maximum, with frequent lifting and/or carrying of items weighing up to 10 pounds
- Normal tasks may involve exposure to blood, bodily fluids, or tissue
- May involve 24-hour on-call responsibilities
- Reliable transportation for frequent travel
