Job Summary
Triage phone calls and ensure callers' needs are met efficiently using excellent customer service skills. Provide and manage direct care to patients and families incorporating psychosocial, spiritual, cultural, physical, and biological components, and appropriate nursing intervention and follow-up. Gather necessary information and required documentation from appropriate sources to ensure complete, current, and accurate documentation of patient information and data. Accurately enter data into information tracking systems, including entering and tracking patient referral files in electronic medical records. Provide reassurance to patients and families, assist in finding solutions to their questions, and coordinate or complete in-person visits when needed. Communicate with the Interdisciplinary Group and document patient/family contact information. During emergencies, report to designated locations to ensure continuity of services.
Required Qualifications
- Current Affiliate state or multi-state license to practice as an RN
- Ability to apply for/obtain multi-state RN license within 30 days of hire, if not already a holder of a multi-state license
- Minimum of one (1) year nursing experience
- Active BLS for healthcare professionals from the American Heart Association or Red Cross
- Ability to effectively communicate in English (verbal and written)
- Ability to work shifts to accommodate extended hours of operation as scheduled
- Ability to perform sedentary work with frequent interruptions
- Ability to perform the essential functions and physical requirements lifting patients and/or equipment, bending, pushing/pulling, kneeling)
Desired Qualifications
- hospice or hospital experience
- bi-lingual in Spanish
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
