Job Summary
Conduct thorough nursing assessments, evaluate care plans, and develop initial Plans of Care for the interdisciplinary team. Identify educational needs for patients and families while demonstrating clinical competency in symptom and pain control. Coordinate patient care with attending physicians, support staff, and community partners, and mediate problems to ensure continuity. Complete medication reconciliation at every visit, manage preauthorization for external providers, and prepare families for discharge or the dying process. Document all visits timely and participate in on-call rotations, weekend shifts, and holiday coverages. Serve as a nurse preceptor and collaborate with clinical supervisors to arrange coverage and improve department functions.
Required Qualifications
- RN with current RN licensure in the state of Kentucky and/or Indiana as required by designated position, or the ability to obtain required license/s upon offer of position
- Must have a valid driver's license
Desired Qualifications
- BSN desired
- Certification in hospice/palliative care preferred
- One year of nursing experience preferred
- Knowledge of palliative care helpful
- Use of technology-based tools, including electronic medical record, strongly preferred
