Job Summary
Coordinate Individual Support Plans, update documentation, and manage intake and transition processes for people supported in Day Support Activities. Monitor progress toward ISP goals, review referral packets, and track service authorizations while maintaining accurate records of medical, behavioral, and safety updates. Serve as a communication link among families, providers, case managers, and internal teams to ensure continuity of services. Provide direct on-the-floor support for activities of daily living as needed and prepare clear, timely reports. This role requires a Bachelor's degree and two years of case management experience, with a focus on person-centered human services.
Required Qualifications
- Bachelor's degree in a related field
- at least 2 years of case management experience
- 4 years of relevant experience with foundational knowledge of social services
- Proficiency with software applications and tracking systems, including Microsoft Office (Excel, Teams, Outlook)
- Ability to meet all background check, reference, and driving requirements

- Experience hosting and participating in virtual meetings and training modules
