Job Summary
Assess, develop, and implement programming for participants covering job readiness, independent living, education, and support needs. Develop and monitor individualized development plans while maintaining accurate case files in compliance with agency, CARF, and contract requirements. Provide direct support based on participant needs, organize interdisciplinary meetings, and build relationships with families and funding sources. Manage community resources to enhance communication, socialization, and daily living skills. Ensure the vehicle is clean, stocked, and maintained according to logs and safety compliance standards. Complete annual van training and adapt to change through effective project management and problem-solving. Serve as a community ambassador by participating in volunteer opportunities as schedules permit.
Required Qualifications
- Two years of college education or experience equivalency
- Minimum of 1 year experience
- Valid driver's license and driving record which meets insurance, company criteria
- Own vehicle or reliable transportation
- First aid/CPR and Medication Management certification
- Work experience in a recreational or human service field
- Ability to lift and/or move up to 10 pounds regularly
- Ability to lift and/or move up to 20 pounds frequently
- Ability to lift and/or move up to 50 pounds occasionally
- Ability to move about to accomplish tasks
- Ability to raise objects from lower to higher position or move objects horizontally
- Ability to stoop, kneel, crouch or crawl
- Ability to reach upwards or downwards to retrieve objects
- Ability to remain stationary for extended periods of time
- Ability to use hands repetitively
- Ability to use vision
- Ability to hear
- Ability to communicate on phone and in person
- Travel required
