Job Summary
Monitor program implementation, conduct regular assessments, and track participant progress to ensure adherence to policies and OPWDD reporting standards. Implement individualized life plans and staff action plans that promote independence, social skills, and community inclusion through structured activities and positive reinforcement. Maintain accurate documentation of daily services and incidents in electronic health systems while managing petty cash and activity budgets in compliance with agency procedures. Provide safe transportation for participants throughout the day, conducting pickups and drop-offs while maintaining vehicle logs and reporting maintenance or accidents. Deliver direct support services in center-based and community settings to help adults with developmental disabilities achieve their personal growth goals.
Required Qualifications
- High-school diploma
- Valid NY/NJ/CT/PA Driver's License with 6 months of experience and 6 or less points
- CPR & SCIP certification
- High School / G.E.D.
