Job Summary
Day Habilitation Coordinator drives the development, coordination, and oversight of individualized program plans for individuals served by Upstate Caring Partners’ Day Habilitation programs. Acts as Treatment Team Leader coordinating services with Nursing, Behavioral Services, Therapy, families, and outside agencies; develops and monitors resident day habilitation staff action plans and person-centered program plans; ensures monthly summaries and widget reviews are completed; trains staff on service plans and documentation; ensures integration of services across programs; conducts direct observation for monitoring; performs monitoring visits; conducts quality assurance reviews; maintains active participant charts to support regulatory requirements; participates in leadership activities and committee work; facilitates positive communication with families and supports; ensures compliance with government and agency regulations and operating standards; requires establishing a work environment aligned with agency culture and mission; travel is required and a NYS Driver’s License is required.
Required Qualifications
- A bachelor's degree from an accredited program in a human services field (including, but not limited to: sociology, special education, rehabilitation counseling, and psychology) as well as any related academic disciplines associated with the study of human behavior, human skill development, humans and their cultural behavior, or any other study of services related to basic human care needs or the human condition, or who holds permanent certification from the New York State Education Department in a field of education; AND Specialized training or at least one year of experience in treating or working with persons with developmental disabilities.
